registration Requirements
Parents must submit the following to the school office before admission can be granted

  • Identification and Emergency Information – LIC 700
  • Child’s Preadmission Health History – Parent’s Report – LIC 702
  • Child’s Preadmission Health History – Physician’s Report – LIC 701
  • Copy of Birth Certificate/Passport
  • Consent for Medical Treatment – LIC 627
  • Copy of Current Immunizations (staff transfers info. to Blue Cards, PM 286)
  • Signed and dated receipt of Notification of Parent’s Rights – LIC 995
  • Signed and dated Personal Rights – LIC 613A
  • Signed Admissions/financial Agreement
  • Non-refundable annual registration fee
  • Post-dated checks for all months child attends until the end of the school year /direct deposit / payment for whole year.
Elementary School
  • Completed Elementary School Application
  • Copy of the child’s Birth Certificate
  • Copy of the child’s up to date immunization card
  • Identification & Emergency Information Form
  • Copy of the child’s
  • Report of Health Examination form (KG only)
  • Admissions Agreement
  • Non-refundable annual registration fee deposit / payment for whole year.
Immunization Requirements
All students enrolled at Folsom Educational Academy must provide a copy of current
immunizations or a signed exemption due to personal or religious beliefs. California law
requires that all private schools, child care centers, day nurseries, nursery schools, and
development centers submit an annual immunization report on children enrolled in child care
centers (CDPH 8018) every year. Children must have the following immunizations:

  • Polio doses 3+
  • DTP/Td Doses 4+
  • MMR Dose(s) 1+
  • HIB Dose(s) 1+
  • HEP B Doses 3+
  • Varicella (chicken pox vaccine) Dose(s) 1+
